Horizon Human Services offers habilitation assistance to persons with developmental disabilities through a contractual partnership with Arizona’s Division of Developmental Disabilities. These services embrace a person-centered philosophy, which emphasizes individuality, personal choice, inclusion and integration.


Services are designed to provide participants with an opportunity to learn, an opportunity to be integrated into their communities, and an opportunity to experience measurable success. Through the Community Integration Day Program, persons with significant developmental disabilities are provided with options and choices.  The participants are given control of the decision making process with staff providing individualized support and assistance.  This participant choice process creates a large amount of community inclusionary activities.

Geographic Areas:

Horizon Human Services currently provides habilitation services through its Community Integration department in Pinal, Gila, Santa Cruz and Cochise counties.  Additionally in the Globe - Miami area Horizon Human Services provides in home habilitation supports and assistance with educational objectives in cooperation with local schools.  Opportunities for additional services in Gila (including the Payson area), Santa Cruz, Cochise and Pinal Counties are in the developmental stage.  Horizon is also evaluating the needs for similar services in other communities throughout Arizona.
