Meet Me Where I Am is a program that holds the child and his/her family as the center of care. It is designed to meet the child and the family
exactly where they are, providing the services that will most benefit the child. This program focuses on supporting the family and providing
rehabilitation services to enrolled children and their families. The objective is to keep the family unit through difficult times and complex
needs. The family participates with a team of workers, counselors, paraprofessionals, etc., in deciding the services needed and the
preferences of service.
Ultimately, this program ensures that the increase in services will result in keeping the children living at home with their families, or in
complex cases, to return to a community-based place to live. Each participant is individually assessed, served and supported with a personally
tailored plan of care and the services will be provided at home, school, office or where the needs of each participant requires.
